4. SMS/Text Messaging Program

Program Name: Neurohealth SMS Notifications

What we send: If you opt in to receive text messages from Neurohealth, we may send you appointment confirmations, appointment reminders, rescheduling notifications, pre-visit preparation instructions, and general office announcements related to your care at our center.

How you opt in: You may opt in to our SMS program by actively checking the SMS consent checkbox on our website booking form, completing a paper or electronic intake form that includes SMS consent, or verbally requesting text message communications during a phone call or office visit. Your consent to receive text messages is entirely voluntary. Providing your phone number and consenting to SMS is not required to book an appointment or receive services from Neurohealth.

Message frequency: Message frequency varies based on your appointment schedule. You may receive up to 8 messages per month.

Message and data rates: Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

How to opt out: You can cancel the SMS service at any time. Just text "STOP" to the short code or phone number from which you received messages. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carriers are not liable for delayed or undelivered messages.

5. No Sharing of Mobile Information

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties, excluding aggregators and providers of the Text Message services.

Important notice about SMS: Standard SMS and MMS text messages are not encrypted and are not a fully secure or confidential method of communication. There is a risk that text messages may be intercepted by, or accessible to, unauthorized third parties, including wireless carriers and other intermediaries.

What we will never include in text messages: Neurohealth will never include Protected Health Information (PHI) in text messages. This means we will never send your diagnosis, treatment details, neurofeedback session results, clinical records, test results, medications, or billing details via text message.

What our text messages may include: Our text messages are limited to appointment date and time confirmations, appointment reminders, rescheduling notifications, general pre-visit instructions (such as "please arrive 10 minutes early"), and general office announcements.

Your rights: You have the right to request that we communicate with you through an alternative method or at an alternative location. If you prefer not to receive text messages, you may opt out at any time by texting STOP, or by contacting our office to request an alternative communication method.

8. How We Protect Your Information
